Registration
New patient appointments are scheduled on Monday mornings to facilitate patient participation in education class beginning Monday afternoon. Upon arrival, you will need to present your insurance identification card and insurance referral form if applicable. We will need a current copy of your insurance card at every visit. Your office visit insurance co-pay is due at registration. Please see "Financial Policy" for further information.
Registration forms must be fully completed prior to your physician visit. We ask that a self-care history form be completed at each visit if you have diabetes or are following a special meal plan. This information nables us to provide the quality care you deserve. Waiting Room We strive to provide patient comfort and adequate patient seating accommodations. Patients are seen as scheduled by appointment with provider, not by patient arrival time. We encourage a family member to accompany you, or both parents accompany a minor patient if possible. However, please limit the number of additional family members to allow adequate seating for other patients.
Physician Visit At Mid-America Diabetes Associates we are proud to be one of the few in the nation to offer a Certified Diabetes Educator at each and every visit with us. It is an opportunity for you to receive additional individualized attention with concentration on meal plans, exercise, and other daily concerns affecting your diabetes. Your initial visit may take two to three hours allowing time with the Physician and Certified Diabetes Educator. Diabetes Education Day Class If you are newly diagnosed or have not received previous diabetes education we encourage you to attend our day class following your initial appointment.
